PRICING

The pricing structure was created based on the higher needs of the infants and younger children. The younger your child is, the more times they will be seen for wellness visits. Younger children typically get sick more often in their first 5 years of life as well. We will try to prevent illness as much as possible, but we factored this into the pricing so that you don’t have to worry about how many times you will need to bring your child in to be checked on or how you are going to be able to reach your nurse practitioner with questions or concerns. This is all covered in the annual fees.

0-2 year old

$1800/Year
  • This includes home visits in the first 3 months of life.
  • All well exams monitoring growth and development including:
  • 3 days
  • 2 weeks
  • 6 weeks
  • 4 month
  • 6 month
  • 9 month
  • 12 month
  • 15 month
  • 18 month
  • This also includes any further needed appointments for weight checks or sick visits plus direct access to your nurse practitioner with questions or concerns.

2-5 year old

$1500/Year
  • This includes all well exams and sick visits or other concerns
  • Well exams:
  • 2 year
  • 2.5 year
  • 3 year
  • 4 year
  • 5 year
  • This also includes any further needed appointments for weight checks or sick visits plus direct access to your nurse practitioner with questions or concerns

6-7 year old

$1500/Year
  • This includes yearly well exams, sports physicals, sick visits, and any follow-up visits needed.
  • This also includes any further needed appointments for weight checks or sick visits plus direct access to your nurse practitioner with questions or concerns.

18-21 year old

$900/Year
  • This includes yearly well exams, sports physicals, sick visits, and any follow-up visits needed.
  • This also includes any further needed appointments for weight checks or sick visits plus direct access to your nurse practitioner with questions or concerns.

4th Trimester Package

$1000
  • This includes:
  • Prenatal meet and greet visit (30 min)
  • In-home visits up to 10 miles from the clinic for the first 3 months of life. Up to the next 10 miles there will be an additional $75 per visit). If you live outside of 20 miles from the clinic we are unable to accommodate in-home visits. If you prefer to drive to the clinic we can arrange this as well for no further fee.
  • 3 day check-up post delivery
  • 2 week well visit (2nd Newborn Screen at additional cost)
  • 6 week well visit
  • 3 month transfer of care visit
  • Up to 4 more in-home visits per month as needed for: labs, weight checks, feeding support. Direct access to your nurse practitioner via text/email, phone, or video as needed.
  • *We are not equipped to give vaccines at this time, if you would like your first set of vaccines you will be given instructions to go to the health department or a traditional pediatric office for these*
